Application Process

Applicants will have to collect required documents and send them to our office with any trackable mailing service (i.e., FedEx, USPS, UPS, etc.). Applicants can also drop the documents off at our office in person (4600 E Washington Street #300, Phoenix, AZ 85034). When your visa has been approved, we either call you for pick-up or we mail the passport with the visa back to you right away.

Required Documents

1. One of the following documents:
   1. Original passport or a copy of the passport.
   2. A copy of the Re-entry Permit, or a copy of the green card (for overseas Vietnamese only).

   It is required that the above-mentioned documents must have at least 1 month of validity prior to the date of exit from Vietnam and/or 6 months of validity to meet airlines’ requirements.

2. Visa Application Form:

If you have any problems downloading the form, please contact this office. (Note: if you make a mistake on the form, please complete another form. No white-out/correction fluid or cross out is allowed.)
3. One recent passport-type color photograph (2″x2″). Must be able to see all of the face and printed clearly.
4. Self-addressed stamped envelope:
   For security concerns, using a trackable mailing service is highly recommended. If you choose to use USPS Regular Mail or Priority Mail without confirmation to send passport(s), we are not responsible for lost mail and lost passport(s).

Processing Fee and Payment Method:

We accept personal checks, business checks, cashier’s checks, money orders, or credit card payments (All major credit cards accepted). Make check or money order payable to: Global Travel.
